Pediatric Home Care Nursing Services

Our State licensed and pediatric trained nurses provide home care for medically fragile children from 0-20 years of age. Also known as Private Duty Nursing (PDN), this skilled nursing service can be provided to our patients for up to 24 hours daily, depending on the child’s need as directed by the primary physician overseeing the child’s medical care.

Medical Social Work Services

Our Medical Social Workers (MSW) facilitate long-term planning to promote independence and provide opportunities to take advantage of various community resources designed to assist patients with their personal concerns and help alleviate their troubles.

The Field Registered Nurse, who is/may be the case manager of the Home Health Team, is responsible for the nursing care of patients assigned to them and directs the Home Health Aides in quality patient care. The Field Registered Nurse is responsible for assessing patient and family needs to promote the best care the Home Health can give for recovery and rehabilitation. The Field Registered Nurse provides nursing services within the scope of practice authorized by the license issued by the State for a Registered Nurse.
